动物医院患者满意度调查合同编号:__________AnimalHospitalPatientSatisfactionSurveyBETWEEN[AnimalHospitalName](Hereinafterreferredtoas"AnimalHospital")AND[PatientName](Hereinafterreferredtoas"Patient")RECITALSWHEREAS,PatientisavaluedclientofAnimalHospitalandhasavailedthemselvesoftheveterinarycareandservicesprovidedAnimalHospital;WHEREAS,AnimalHospitaldesirestogatherfeedbackfromPatientsregardingtheirexperienceandsatisfactionwiththeservicesprovided,inordertocontinuouslyimprovethequalityofcareandservicesoffered;NOW,THEREFORE,inconsiderationofthemutualcovenantsandagreementscontainedherein,thepartiesheretoagreeasfollows:1.ScopeofServices1.1AnimalHospitalagreestoconductapatientsatisfactionsurveyamongitspatients,includingthePatient,togatherfeedbackonthequalityofcare,services,andoverallexperienceprovidedAnimalHospital.1.2Thesurveyshallbeconductedinamannerthatisfair,unbiased,andconfidential,ensuringthattheresponsesprovidedthepatientsareanonymizedandprocessedcollectivelyforstatisticalanalysispurposesonly.2.SurveyConductandTimeline3.ConfidentialityandDataProtection3.1Bothpartiesagreetomaintaintheconfidentialityofthesurveyresponsesandanyothersensitiveinformationexchangedbetweenthem,unlessrequiredapplicablelaworregulationtobedisclosed.3.2AnimalHospitalshallensurethatallpersonaldatacollectedfromthePatientandtheiranimalisprocessedinaccordancewithapplicabledataprotectionlawsandregulations,andthatappropriatesecuritymeasuresareinplacetosafeguardsuchdatafromunauthorizedaccessordisclosure.4.UseofSurveyResults4.1AnimalHospitalagreestousethesurveyresultssolelyforthepurposeofimprovingthequalityofcareandservicesprovidedtoitspatients.5.Indemnification5.1AnimalHospitalshallindemnifyandholdharmlessthePatientfromanyandallclaims,damages,losses,orexpensesincurredthePatientasaresultofthebreachofanytermorconditioncontainedhereinoranyactoromissionofAnimalHospitalinconnectionwiththesurvey.6.GoverningLawandDisputeResolution6.1ThisAgreementshallbegovernedandconstruedinaccordancewiththelawsof[Jurisdiction].6.2AnydisputesarisingoutoforinconnectionwiththisAgreementshallberesolvedthroughbindingarbitration,inaccordancewiththerulesofthe[ArbitrationInstitution],andthedecisionofthearbitrator(s)shallbefinalandbindingupontheparties.7.EntireAgreement7.1ThisAgreementconstitutestheentireagreementbetweenthepartieswithrespecttothesubjectmatterhereofandsupersedesallprioragreements,understandings,negotiations,anddiscussions,whetheroralorwritten,oftheparties.INWITNESSWHEREOF,thepartieshaveexecutedthisAnimalHospitalPatientSatisfactionSurveyAgreementasoftheEffectiveDate.ANIMALHOSPITAL:By:___________________________Name:Title:Date:PATIENT:By:___________________________Name:Date:一、附件列表:1.AnimalHospitalPatientSatisfactionSurveyForm2.ConfidentialityAgreement3.DataProtectionPolicy4.ArbitrationAgreement5.ListofPermittedUseofSurveyResults二、违约行为及认定:1.Failuretoconductthepatientsatisfactionsurveywithintheagreedtimeline.2.Breachofconfidentialityandunauthorizeddisclosureofsurveyresponsesorpatientinformation.3.Useofsurveyresultsforpurposesotherthanimprovingthequalityofcareandservices.三、法律名词及解释:1.AnimalHospital:Aveterinarycarefacilityprovidingmedicalservicestoanimals.2.Patient:Theindividualorentityresponsiblefortheanimalreceivingveterinarycare.3.PatientSatisfactionSurvey:Aquestionnairedesignedtogatherfeedbackfrompatientsregardingthequalityofcareandservicesprovidedtheanimalhospital.4.Confidentiality:Theobligationtokeepinformationsharedbetweenthepartiesprivateandnotdiscloseittounauthorizedparties.5.DataProtection:Thelegalframeworkgoverningtheprocessingandprotectionofpersonaldata.四、执行中遇到的问题及解决办法:2.Breachofconfidentiality:InitiateadiscussionwithAnimalHospitaltoaddresstheissueandtakeappropriatemeasurestoensureconfidentialityismaintained.3.Unauthorizeduseofsurveyresults:ReviewtheArbitrationAgreementandseeklegalrecourseifAnimalHospitalusesthesurveyresultsforunauthorizedpurposes.五、所有应用场景:1.AnimalHospitaldesirestogatherfeedbackfrompatientstoimprovethequalityofcareandservices.2.PatientiswillingtoprovidefeedbackontheirexperiencewithAnimalHospital'sveterinaryserv
