6、y bothparties prior to the term in ati on date.)第二条：服务项目Article 2: Service Items一、 乙方须告知甲方有关法令、风俗、民情、薪资、福利、法定费用扣缴及工作权益 等相关信息。Party B shall no tify Party A of laws an d/or regulati ons, folk customs, salaries, welfares, legalexpe nse withholdi ng and work ing rights and in terests in the R.O.C.二、 乙方应

7、协助安排接送甲方至雇主指定工作处，及甲方聘雇关系终止或聘雇许可期 间届满返国。Party B shall render assistance in making arrangements for pick up/delivery of PartyA to the work location designed by employer and for the repatriation upon the expirati on of thevalidity of the employme nt permit or term in ati on of the employme nt.三、 乙方应每一个

8、月电话联系或访视甲方一次，提供翻译、咨询服务、协助排解工作 压力与生活适应及管理等事项Party B shall contact or visit Party A one time per month by phone or in person to provideservices of translation, counseling, pressure relief, living accommodation, management, etc.四、乙方须协助甲方与雇主沟通、协调、纠纷排解，并将雇主之工作规则及生活管理 事项翻译成甲方母国之文字让甲方熟悉与了解。Party B shall as

9、sist Party A in communicating, coordinating and mediating the dispute withEmployer as well as translate the rules of work and daily activity management set up by theemployer into Party As mother language, so that he/she can familiarize and understand.五、乙方于甲方入国后，依入出国及移民法相关规定期限，协助甲方至内政部入出国 及移民署制作指纹纪录，

10、并办理居留业务。After Party A enters the R.O.C., Party B shall assist Party A in going to the NationalImmigration Agency, Ministry of the Interior to take fingerprints and to register residence withina timeframe specified in the Immigration Act.六、乙方应告知甲方办理健检及核备程序之情形。Party B shall notify Party A of the proce

11、sses for the medical checkups and for submitting theresults to the competent authority of the local government for reference.七、甲方发生意外事故时，乙方须协助处理之事项如下：When Party A is involved in accident, the assistance shall be rendered by Party B as follows:（一）甲方重伤或死亡，乙方提供免费善后协商服务，协助甲方之雇主联络甲方之家 属及协助来台处理善后事宜，或协助甲方返

12、国及将甲方遗体及其私人物品运送返国。In the event Party A suffers seriously injury or death in an accident, Party B shall provide freenegotiation services; assist the employer of Party A in contacting the family members of PartyA to come to Taiwan for handling follow-up works; or assist with the repatriation of Party

13、A andrepatriate the remains of the Party A and his/her personal belongings to his/her country oforigin.二）乙方因协助甲方所衍生之费用，应由甲方负担Party A shall bear the expe nses in connection to the assista nee provided by Party B.八、甲方触犯法令或因故遭遣返时，乙方应协助甲方办理出国事宜。In the eve nt of repatriati on as a result of Party As viol

Party Bshall assist Party A in leaving the country.第三条：服务费：Article 3: Service Fees第一年每月新台币 _ 元，第二年每月新台币 _ 元，第三年每月新台币 元NT$_ per month for the first year, NT$_per mon th for the sec ond year andNT$_ per mon th for the third year.第四条：收费及退费方式Article 4: Expe nse and Refund一、

15、费用给付方式：Payment:（一）甲方应于每月 _ 日前，以现金给付汇款转帐方式其它 _ 等方式支付乙方服务费。Party A shall pay the service fees to Party B by cash, wire tra nsfer or others_beforeper mon th.（二）甲方应负担之健康检查规费、居留证规费、展延居留证规费。本款代办项目之 规费应于乙方办理时给付。Party A shall pay for the expe nses of health exam in ati on, reside nee permit and exte nsion.T

16、he expe nses un der this subsect ion shall be paid to Party B at the time services are rendered.（三）乙方收取服务费时，应掣给收据或发票，并保存收据或发票存根五年。Receipt or in voice shall be issued by Party B while collect ing service fees. Each receipt or invoice shall be maintained for a period of 5 years.二、乙方应依第二条第二款规定，协助安排接送甲方

17、，不得向甲方收取接送所需之交 通费用Party B shall, in accordanee with Article 2(2), render assistanee in makingarran geme nt for party B to be picked up and shall not impose any tran sportati on fees forpick-up service on Party A.第五条：甲方之义务Article 5: Party As Obligati on甲方应于乙方依本契约提供服务后，按期缴交服务费。Party A shall make timel

18、y payme nt for the service fees to Party B after the services are rendered un der this Con tract.第六条：乙方之义务Article 6: Party Bs Obligati on一、本契约订定前，乙方应对甲方详细说明契约条款内容。Party A shall be well-i nformed of the terms and con diti ons of this Con tract by Party B beforemaki ng this Con tract.二、乙方应依契约规定，履行乙方应办

19、事项。Party B shall perform the services set forth in this Con tract.三、乙方不得收受规定标准以外之费用。Party B shall not charge for additi onal fees other tha n those specified in the releva nt regulations.第七条：契约订定、变更、终止及损害赔偿Article 7: Establishment, Modification, Termination and Compensation一、契约自甲、乙双方订约日起生效，契约内容的变更或记

20、载事项的增删，非经甲、 乙双方书面同意，不生效力。This Con tract takes effect on the day sig ned by Party A and Party B. However, conditionsor circumstances not attributable to both Parties or attributable to the period whe n Party A isbeing placed in the shelter shall be exempted from this provisi on.四、 甲、乙双方就契约所生义务之不履行或延迟履行，而致他方受有损害时，应负损 害赔偿责任。Whe n either Party fail or delay to perform its obliga

24、chments published by Party B for engaging in oversea man powerage ncy operati on shall be deemed as a part of this Con tract.一第九条：保密条款Article 9: Con fide ntiality一、乙方对于甲方之个人数据应予保密，并不得为不当使用。Party B shall keep Party As pers onal in formati on con fide ntial, and not misuse that informati on in any way

25、.二、乙方违反前款规定致甲方受有损害者，应负赔偿责任。For Party Bs violation of the previous provision that cause the damage or loss to Party A,Party B shall be liable for reimburseme nt.第十条：通知之送达处所Article 10: Notice一、 双方意思表示之通知，均以契约书所载之地址为送达处所，变更时应以书面通知 他方。All no tices or other com muni cati ons give n by one Party to the ot
