PurposeTo give a guideli ne for reporting and classify ing injury, ill ness and in cide nt in a timel

ResponsibilityAll injuries an
6、d incidents must be reported on-site as soon as possible. Injury and incident classification must be completed within 48 hours (LWC within 24 hours) in order to meet the off-site reporting guidelines.4.1 IndividualsEvery DSPC employee or contractor must report all unsafe conditions, injury, illness
7、and incident symptoms to his/her line supervisor or area supervisor immediately by telephone, E-mail or other communication methods. Acting promptly to eliminate or prevent unsafe conditions. Working in a safe manner. Following safety procedures.4.2 Line Supervisor or Area Supervisor of Each Dept.Li
8、ne supervisor or area supervisor must report injuries, illnesses and incidents to S&OH Dept. and DSPC plant manager as soon as possible by telephone, E-mail or other communication methods. Meanwhile, they must take actions to form investigation team, write preliminary report and finish final report
9、based on the final classification both in English and Chinese. Communicate the unsafe condition, injuries, illnesses and incidents to his line staff and prevent reoccurrence.4.3S&OH Dept.S&OH Dept. must report injuries, illnesses and incidents to DSPC plant manager as soon as possible by telephone,
10、E-mail or other communication methods.S&OH Dept. will provide helps on investigation and classification. The final report must be filed and distributed on-site by S&OH Dept. S&OH is also responsible for site-wide communication on injuries, illnesses, and incidents to prevent reoccurrence.4.4DSPC Pla
11、nt ManagerPlant manger or designee will give guideline and approval on classification and offsite report, define the off-site report distribution list and send the off-site report to country leader, SBU, GSB, CORE (or function) leader and A/P SHE Directorimmediately. Country leader to activate A/P C
12、risis Center at Level 1 (minimum). SBU (or function) leader to inform A/P President and SBU VP/GM. A/P SHE Director will inform SHE Excellence Center.4.5 GREP Dept. - Local Government ReportAccording to the Local Laws or regulations, we will report injuries, illnesses and incidents to local governme
13、nt. GREP Dept. will define off-site distribution list, and send report to related local government with approval from site GM.4.55.5.1IHS (Integrity Health Service)Injuries and IllnessDefinition (Ref. S35G)On-The-JobInjury - any injury such as a cut, fracture, sprain, amputation, etc., which results
14、 from a work-related incident or from an exposure involving an incident in the work environment. Injuries typically result from something that happens in one instant.Illnesses- Any disorder not resulting from an injury, caused by environmental factors associated with employment. Illnesses are record
15、able by definition (No first aid case illnesses).Severity categories -categories that denote the seriousness and impact of an injury or illness onhealth and work. They are LWC, RWC, MTC and FAC.Lost Work Cases (LWC) - If an employee is unable to work on a subsequent scheduled shift because of a work
16、-relatedinjury or illness? the case is an LWC. Restricted Work Cases (RWC) - An RWC is a case in which a work-related injury or illness prevents the employee from working acomplete shift or from doing any tasks that are part of his or her regularly scheduled job but whichdoes not result in lost work
17、days.Medical Treatment Cases(MTC) - An MTC is a case for which medical treatment is necessary but which does not result in lost workor work restrictions.First Aid Cases (FAC) - A minor injury that needs only simple treatment and does not need follow-up treatment by a healthcare professional is an FA
18、C. A case can be classified as an FAC even if the first aid isadministered by a health care professional. Total Reportable Cases (TRC) - The total of MTCs + RWCs + LWCs Condition categories- four categories that denote whether an injury or illness was related to anevent and whether it is an accounta
19、bility metric. They are event-related (ER) conditions, otherevent-related (OER) conditions, nonevent-related (NER) conditions, and other nonevent-related(ONER) conditions.Event-related (ER) conditions- injuries and illnesses that result from a specific workplace event(e.g., a fractured knee from a f
20、all or a sprained back from lifting).Other event-related (OER) conditions- injuries or illnesses that are event related but that donot have a clearly established work relationship, are of low severity, or are not preventable byDuPont, yet may be recordable by regulatory rules.5.26.Nonevent-related (
21、NER) conditions- ill nesses likely caused or aggravated by the work pl ace but not related to a sp ecific eve nt (e.g., carpal tunnel disorder).Other nonevent-related (ONER) conditions- ill nesses that are not related to a sp ecific eve ntand do not have a clearly established work relatio nship, are
22、 of low severity, or are not p reve ntableby DuPont, yet may be recordable by regulatory rules.Off-the-job injury - Any harm or damage to the body, or body system, result ing from viole nee, poisoning, or other exter nal causes, which does not arise out of and in the course of employment and results

Major distribution incidents with significant public impact or potential for media attention should be reporte
40、d immediately to the country leader who should activate the A/P Crisis Center at level 1 (minimum).For additional information on incidentreporting and classification, refer to DuPont SHE Guideline S11U and S2U.12.
