1、Word - 24 -承包商安全制度 Contractor Safetyx 现场承包商平安Contractor Safety Standard Contractors must develop location management systems that provide reasonable assurance that on-site contractors(sub-contractors) perform their work in compliance with applicable governmental and location environmental,safety and

2、 health requirements. All contractors performing work on factory property should operate in such a manner as to not endanger factory employees, physical property, the public, the environment or themselves. Minimum Requirements 1. Contractors must establish specific responsibilities as applicable for

3、 the users of sub-contractor services, such as maintenance, administration, and purchasing. Responsibilities should be appropriate for the organization at the location. 2. Specific ESH requirements should be determined for each sub-contractor/contract job based on the work that the sub-contractor is

4、 to perform. 3. Sub-contractors should be selected based on their ability to meet applicable governmental and location ESH requirements and Nikes ESH performance criteria. 4. Sub-contractors who provide or perform process equipment maintenance, provide specific non- maintenance services to operation

5、s or perform construction work should be pre-qualified. Pre-qualification of other contractors should be determined by location management based on the level of risk associated with the work that the contractors are to perform. 5. The sub-contractor should provide documentation to verify that the co

6、mpany can comply with applicable ESH requirements and performance criteria. The level of verification should be established by location based on experience with the contractor and on the work to be performed by the contractor. 6. Pre-job reviews should be held with the sub-contractor to provide a lo

7、cation orientation and to assure that sub-contractor ESH requirements are understood. 7. Contractors must monitor sub-contractor compliance with ESH requirements on an ongoing basis. The level of monitoring depends on the level of risk associated with the work that the contractor is performing 8. Al

8、l sub-contractor employers and employees are to attend an on-site Environmental, Safety Health orientation before working on site. Training and/or review may be warranted to conform to Nikes Chemical Hazard Communication, Control of Hazardous Energy, Confined Space Protection, and Maintenance Safety

9、 CLSs. The responsible project manager or engineer is to walk sub-contractor employers and employees through the factory and provide an explanation of the evacuation plan and alarm systems. The project manager or engineer is also to walk the sub-contractor employers and employees through the specifi

10、c areas in which they will be working and review any environmental, safety, or health hazards and protective measures associated with that area. The designated ESH Representative is to be included in this walk-through. 9. No one under the influence of drugs or alcohol is permitted on factory propert

11、y. Drugs or alcoholic beverages shall not be brought onto the property. 10. Horseplay and recklessness is not permitted and will not be tolerated as it endangers sub-contractor and factory employees and property. 11. A hot work permit is to be obtained from the maintenance department before any hot

12、work is performed. Hot work includes any welding, burning, open flames, or torch cutting operations. A fire watch with a proper extinguisher is to be placed on hot work tasks until the operation is complete. 12. Chemicals are not to be brought on site without prior review from the contractor. Chemic

13、als will be reviewed for their hazard potential and disposal requirements. All chemicals must be properly contained in specified containers. 13. Transport of chemicals through any part of the facility without prior consent from the ESH Representative is prohibited. 14. Grounding and bonding procedur

14、es must be followed for all flammable materials. 15. 18、的设备设施. Contractor Safety and Health Implementation Guidelines Purpose: To provide guidelines for the implementation of the (FACILITY) Contractor Safety and Health (SH) Policy. These guidelines may be used to develop location management systems that provide reasonable assurance that on-site contracto

19、rs perform their work in compliance with applicable governmental and location health, safety and environmental requirements. Guidelines: These guidelines are structured to provide direction for implementing the specific elements of the (FACILITY) Contractor SH Policy. Accountability: Location manage

20、ment systems should address the specific accountabilities for managing on-site contractor SH compliance. Specific responsibilities should be established, as applicable, for Location Contract Maintenance Administration, Purchasing, and the users of contractor services. Responsibilities should be appr

21、opriate for the organization at the location. Determining Contractor SH Requirements: Specific SH requirements should be determined for each contractor/contract job based on the work that the contractor is to perform. The Contractor Safety Requirements in this program may be used to determine specif

22、ic requirements. Specific SH performance criteria such as Experience Modification Rate (EMR) and Recordable Injury Frequency Rate (RIFR) should be determined based on annual goals coordinated bye (FACILITY)s Labor Practices Department. Contractor Selection and Approval: Contractors should be selecte

23、d based on their ability to meet applicable governmental and location SH requirements and (FACILITY)s SH performance criteria. Contractors who provide or perform process equipment maintenance, provide specific non-maintenance services to operations or perform construction work should be pre-qualifie

24、d. Pre-qualification of other contractors should be determined by location management based on the level of risk associated with the work that the contractors is to perform. It is recommended that the attached Contractor Safety Qualification Form be used for pre-qualifying contractors. The applicabl

25、e SH requirements and performance criteria should be included with the Qualification Form. Applicable SH requirements and performance criteria should be included as specification in the invitation to bid and reviewed with the bidder on request or in pre-bid meetings. The contractor should provide do

26、cumentation to verify that the company can comply with applicable SH requirements and performance criteria. The level of verification should be established by locations based on experience with the contractor and on the work to be performed by the contractor. Normally, the verification should includ

27、e a check of key requirements. The contractor should sign a written agreement (normally a contract) which commits the contractor to: Abide by applicable governmental and location SH requirements. Meet location SH performance criteria such as EMR and RIFR. Employ or cause to be employed only persons

28、who are fit and skilled in the work to be performed. Use equipment that is safe to operate and meets governmental requirements. Provide (lease, rent or lent to) equipment to the location that meets governmental and location SH requirements. Provide authorized location representatives access to their

29、 work area(s). A written agreement is not required for a contractor whose work on site will not expose the contractor, contractor employees, location employees, or the community to reasonably foreseeable SH hazards. Location should establish specific procedures for emergency work consistent with the

30、 intent of this policy where it may not be practical to follow these guidelines. Pre-Job Reviews: Pre-job reviews should be held with the contractor to provide a location orientation and to assure that contractor SH requirements are understood. The pre-job review can also be used to spot check the c

31、ontractors ability to comply with applicable requirements. Location Representative will discuss with the contractor contact person the scope and time table of the job they are to perform and the hazards thy may encounter such a electrical lines, pneumatic/hydraulic lines, hazardous material, etc. Ou

32、tside contractors sending employees to work at the facility location will be responsible for training their employees on anticipated hazards before starting work. At this time the Outside Contractor Safety Communication Form should be completed. Contractor Performance Monitoring: It is important to

33、monitor contractor compliance with SH requirements on an on-going basis. The level of monitoring depends on the level of risk associated with the work that the contractor is performing. Contractors whose work on-site may expose the contractor, contractor employees, location employees or the communit

34、y to reasonably foreseeable SH hazards should be monitored in more detail. Typical monitoring systems which may be used are as follows: Job Site Reviews A daily walk-thru - SH review should be performed by a location representative(s). This is an informal review that does not have to be documented.

35、Observations and deficiencies identified on these reviews are reported to contractor management. Serious conditions or non-compliance is corrected immediately. Periodic SH Inspections Periodic inspections should be conducted by a location representative to monitor contractor SH performance and addre

36、ss deficiencies. These inspections may be conducted in conjunction with location inspection practices. It is recommended that these inspections be documented. Periodic SH Performance Reviews Meetings should be held periodically by location representatives to review, discuss and evaluate contractor S

37、H performance. Contractors should be represented by a senior contractor representative on site. The following topics should typically be addressed: Performance Accident and Incident Summary Location and Contractor Inspection Summaries Audits - Status Reports SH Issues Periodic Internal Audits Contra

38、ctor SH activities should be included in location audits, or periodic audits of contractor activities should be conducted by location management. Contractor Evaluation Appropriate location representatives should evaluate the SH performance of those contractors whose work may expose the contractor, c

39、ontract employees, location employees or the community to reasonably foreseeable SH hazards. It is recommended that the attached Contractor SH Performance Evaluation form be used. The evaluation may be done at the completion of the job, the expiration of a contract or periodically for long term cont

40、ractors. Contractors Safety and Health Policy Policy Purpose To review contractor qualifications relative to Occupational Safety and Health Loss Prevention and Workers Compensation coverage prior to formalizing a con

47、tract; to review the contractors Loss Prevention efforts once they are on site; and to prevent contractor employee, (Facility) employee and property injury. Policy Statement This procedure is applicable to all (Facility) facilities. Applicable Documents Contractor Safety Qualifications Contractor Sa

48、fety Requirements Policy Procedure 4.1 Contractor Safety Qualifications Form The attached Contractor Safety Qualifications Form must be completed by all prospective contractors as part of the bid process. The form is to be completed and reviewed by (Facility) prior to a contract being formalized wit

49、h a contracting facility. The Qualifications Form outlines the contractors experience in regard to Safety and Health, provides information in regard to their Loss Prevention Programs, and provides information to assure that they are covered by Workers Compensation Insurance. A form is to be complete

50、d for all prime and sub-contractors that will be on site in regard to a contract. 4.2 Pre-construction Conference A representative from the (Facility) facility is to attend the pre-construction conference(s) and review the prime and sub-contractors Loss Prevention Programs relative to the work that

51、the contractors will be performing on site in addition to reviewing the attached “Outside Contractor Safety Communication Form.”. 4.3 Occupational Safety and Health Regulations The contractor is to follow all State Occupational Safety and Health regulations for work performed (See attached Contracto

52、rs Safety Requirements). Additionally, contractors are to follow all (Facility) Safety and Health Procedures, and personal protective equipment requirements as outlined in training and as posted throughout the facilities. 4.4 Training All contractor employers and employees are to attend an on-site S

53、afety Health orientation prior to working on site. Training and/or review may be warranted to conform to OSHAs Hazard Communication, Lockout/Tagout, Permit-Required Confined Spaces, and hot work Permit protocol. The responsible project manager or engineer is to walk the contractor employer and emplo

54、yees through the facility and provide an explanation of the evacuation plan and alarm systems. The project manager or engineer are also to walk the contractor employer and employees through the specific areas in which they will be working and review any safety or health hazards and protective measur

55、es associated with that area. The designated Safety and Health Representative is to be included in this walk-through. 4.5 Weekly Safety Meetings If in-plant contract work extends beyond five days, weekly safety meetings are to be held by all contractors on site to discuss safety issues. The designat

56、ed facility Safety and Health Representative is to attend these meetings and to provide input as to noted safety concerns. 4.6 General Safety Requirements 4.6.1 All contractor employers and employees are to check in daily at (Facility) prior to beginning work on site. 4.6.2 No one under the influenc

57、e of drugs or alcohol are permitted on (Facility) property. Drugs or alcoholic beverages shall not be brought onto the property. 4.6.3 Horseplay and recklessness is not permitted and will not be tolerated as it endangers contractor and (Facility) employees and property. 4.6.4 Hot work permits - a pe

58、rmit is to be obtained from (Facility) prior to any hot work being performed. Hot work would include any welding, burning, open flames, or torch cutting operations.
