1、Operational Safety Supporting Tool操作安全支持工具Vessel Inspections are to be carried out prior to commencing operations upon the vessel. Vessel inspections must be carried out with a representative from the vessel and signed off by the Chief Officer (C/O). Additional safety inspections can be carried out
2、during the shift or as points of work (POW) change, these new inspections must be signed off by the Chief Officer (C/O) or the Deck Officer (D/O). 巡查必须在船上工作开展前进行及必须有一位船代表陪同下进行,检查表必须由大副签署。如在同一班有工作点POW改变了,额外巡查必须进行,本巡查表必须由大副印送往船主或其代表。All unsafe conditions raised during the inspection must be supported

5、ormation of key personnel including Person in Charge码头之主要联络人之姓名及电话包括指令人员1.2 Any outstanding Notice of Deficiencies on vessel database (attach if necessary) 在船只数据库有没有未完成之不足处通知书?Vessel Database shall be kept and regularly updated to monitor and control any outstanding Safety and Environmental non conf
6、ormities / actions raised against the vessel. Vessel Database shall be updated following each vessel inspection carried out. 船只数据库需保存妥当及定期更新,以便监察及控制任何船方未完成之安全及环境违章事宜。待每次巡查后,该船之数据库亦必须更新。1.3 Has the lashing crew undergone a safety induction装卸工人是否已接受了职前安全培训?All employees and contractors representing DP
7、 World must undergo a Safety induction before commencing work at any terminal. 任何新聘任员工或承包商工人在开展工作前必须接受职前安全培训。1.4 Has the lashing crew undergone vessel / lashing safety training装卸工人是否已接受了船上打拆加固之安全培训?All lashing crew must undergo vessel / lashing safety training against their specific job description.
8、 装卸工人必须接受船上打拆加固之安全培训。1.5 Any feedback from Lashing gang or vessel crew装卸工人或船员有没有回馈或意见?Comments / suggestions raised by employees / contractors or vessel crew must be recorded and presented to the Operations and Safety manager. 任何由员工 / 承包商工人 / 船员所提出之评语 /意见必须记录下来及呈交予操作部及安全经理解决。2. Access进出位1 Gangway is
9、 properly landed and clear of cranes gantry path, set correctly and netted舷梯是否正确地摆放?是否不在铁塔桥吊之行机行大机之路线内?是否设置安全网在其下?ILO 7.2.1 (5) A safety net should be rigged where ever a person may fall between the ship and the quay from a means of access to a ship. 按国际劳工组织ILO第7.2.1(5) 所示,安全网必须设置于舷梯与码头之桥边之间,防止人体下坠。
10、ILO 7.2.3 Gangways按国际劳工组织ILO第 所示1. Gangways should be placed at right angles to the ships side and connect the quay with one of the ships decks or with the bottom platform of the accommodation ladder. 舷梯必须与船身呈九十度角,舷梯之最底部与码头之桥边间必须以平平台接联2. A Gangway should舷梯必须:- Have closely boarded walkway at least 5
11、50mm in width; 以550mm之木板铺密;- Be fitted with transverse treads at suitable stepping intervals; 在梯级间配备了横向面;- Be fitted with upper and intermediate guard rails; 配置顶及中护栏;- Be fitted with devices enabling it to be properly secured to the ship; 安全地吊稳;- To be fitted with proper slinging attachments so plac
12、ed that it will balance about the attachment when it is suspended, if a lifting appliance has to be used to placed it in position; 设置合适吊具以稳固及平衡舷梯;- Not normally be used at an angle of more than 30° to the horizontal, or 45° if it is fitted with transverse treads every 500mm or so; 不可超过水平30
13、°,或每隔500mm以45°朝向横向面;- Never be used at an angle of more than 45° to the horizontal; 不可超过水平45°;- Land on a clear and unobstructed area of the quay. 摆放在没有被阻挡的桥边。3. Where a gangway rests on a ships bulwark, safe means of access should be provided between the ships deck and the gangw

A gangway that rest on a quay on rollers or wheels should be in such a way that the rollers or wheels are on a reasonably level surface and not the vicinity of any obstruction or hole that could restrict their free movement. The rollers or wheels should be fitted or gua
15、rded in such a way as to prevent a persons foot from being caught between them and the quay surface, leaving a minimum gap of 50mm. 舷梯之底部须设置轮子或滚轮,以便轮子或滚轮停放在一个合理的水平面上,轮子或滚轮并没有遇上任何障碍物或孔洞,让其在没有限制下自由移动。该轮子或滚轮应安装防护设施,以防止它们与码头桥边之间的空隙可夹伤人脚空隙如少于50mm可以不须理会。5. A gangway should be securely fastened to the ship
16、. One arrangement consists of placing the gangway above the deck or the platform and providing a fastening at the bottom. 稳固地放置在船上。可放置在甲板上或安放在平台上然后稳固其底部。6. Gangways shall be landed securely on quay surface and not obstructing crane travel path or traffic flows稳固地放置在桥面及不在桥吊之行大机之路线内。2.2 Hatch covers a

The devices may be self-locking, so that the locked component can be released only by manual operation.锁定装置应具有足够的强度,以能够承受来自任何摆动的货物

If specially constructed, they may consist of wooden grating or steel plates at least 600mm wide. They should be raised some 100mm above the deck. 各进出途径必须保持

23、re there is no risk of slipping / tripping or hand injuries. 船上人行道之地面及扶手必须为安全状况,以确保没有滑倒或手部受伤之危害。2.6 Lashing points are accessible打拆加固点必须容易获取按国际劳工组织ILO第 所示(1) Shore-side management should ensure that safe access is provided by the ship to any place on the ship where stevedores have to work and that t
24、he place of work is safe. 桥边管理人员必须确保船方在船上提供安全进出通道予装卸工人及安全之工作点。2.7 a) All ladders are in safe condition所有梯具必须处于安全状态Ladders must be in safe condition to climb without the risk of fatigue or rusting. 各梯具必须处于安全状态,没有任何金属疲劳或生锈。2.7 b) Vertical ladders above 3m have safety hoops - (bottom hoop 2m from the b

Edge Protection船缘边之保护3.1 Outboard cells are provided with safety rail
28、s sufficient to prevent a person falling overboard在船最外围的配载位置设置足以防止人体下坠海面之护栏按国际劳工组织ILO第7.4.4 所示1. Hatches at deck level should be protected by coamings of sufficient height to prevent accidental falls into the hold. Coamings should preferably be 1m high. 舱口之开启舱盖必须以足够高度之围板围封,以防止人体下坠入船舱,围板最少有1米高。2. Por
29、t workers should not work on cargos on deck or between that are over an open hatch. 码头工人不准在货箱顶或开启舱盖口旁工作。3. Work in the tween deck are should not normally take place if the hatch is open to the lower hold. If this cannot be avoided, the hatch should be fenced to prevent port workers from falling. 如舱盖

The fencing should be 1m high and may be of suitable wire rope or chain provided that; 护栏必须为一米高或以合适之网丝绳 / 铁链代替- There are means to keep the ropes or chains as taut as possible; 须利用设备将网丝绳 / 铁链拉紧- Wire ropes have sufficient wires per strand to be fle
31、xible, are free from broken wires and any loose ends are fitted with ferrules or other means of protection to prevent injury; 钢丝绳每有足够的钢绞线导线及没有断丝,任何松动的两端都配有套圈或其他保障方法,以防止令作业人员受伤;- Sufficient stanchions are provided. 提供足够的支柱。5. Deck sockets into which stanchions fit should be equipped with locking devi
32、ces and should be sufficiently deep and designed in such a way as to prevent the stanchions from moving unduly out of the vertical or being accidently displaced.支柱在甲板之插座须设置上锁装置,插座有足够之深度,并设计了防止支柱的意外滑出之装置。6. The fencing should form a permanent part of the ships equipment and be kept in place at all ti

36、shing Gear is in good condition to carry out operation safely. 所有吊具必须处于安全状态及安全地操作。4.2 Bins / racks for stowage of lashing gear are provided and accessible at Point of work (POW) 设置大箱/ 架来储存打拆加固工具及在工作点容易获取N/A (All lashing gear and reefer cords properly stowed upon arrival). 不适用所有吊具及冷藏箱电源线在到达码头前,船方已收拾妥
