At least 5 weeks should be allowed after stopping PROZAC before treatment with an MAOI (4, 7.1 Do not use with pimozide due to risk of drug interaction or QT c prolongation (4, 7.9 Do not use with thioridazine due to QT c interval prolongation or potential for elevated thiori

13、dazine plasma levels. Do not use thioridazine within 5 weeks of discontinuing PROZAC (4, 7.9 When using PROZAC and olanzapine in combination, also refer to the Contraindications section of the package insert for Symbyax (4-WARNINGS AND PRECAUTIONS - Clinical Worsening and Suicide Risk: Monitor for c

14、linical worsening and suicidal thinking and behavior (5.1 Serotonin Syndrome or Neuroleptic Malignant Syndrome (NMS-like Reactions: Have been reported with PROZAC. Discontinue PROZACand initiate supportive treatment (5.2 Allergic Reactions and Rash: Discontinue upon appearance of rash or allergic ph

15、enomena (5.3 Activation of Mania/Hypomania: Screen for Bipolar Disorder and monitor for mania/hypomania (5.4 Seizures: Use cautiously in patients with a history of seizures or with conditions that potentially lower the seizure threshold (5.5 Altered Appetite and Weight: Significant weight loss has o

16、ccurred (5.6 Abnormal Bleeding: May increase the risk of bleeding. Use with NSAIDs, aspirin, warfarin, or drugs that affect coagulation maypotentiate the risk of gastrointestinal or other bleeding (5.7 Hyponatremia: Has been reported with PROZAC in association with syndrome of inappropriate antidiur

17、etic hormone (SIADH (5.8 Anxiety and Insomnia: May occur (5.9 Potential for Cognitive and Motor Impairment: Has potential to impair judgment, thinking, and motor skills. 19、sive Disorder, Obsessive Compulsive Disorder, Bulimia, and Panic Disorder: abnormal dreams, abnormal ejaculation, anorexia, anxiety, asthenia, diarrhea, dry mouth, dyspepsia, flu syndrome, impotence, insomnia, libido decreased, nausea, nervousness, pharyngitis, rash, sinusitis, somnolence, sweating,

20、 tremor, vasodilatation, and yawn (6.1PROZAC and olanzapine in combination Also refer to the Adverse Reactions section of the package insert for Symbyax (6-DRUG INTERACTIONS - Monoamine Oxidase Inhibitors (MAOI: PROZAC is contraindicated for use with MAOIs, or within 14 days of discontinuing an MAOI

21、 due torisk of drug interaction. At least 5 weeks should be allowed afterstopping PROZAC before starting treatment with an MAOI (4, 7.1 Pimozide: PROZAC is contraindicated for use with pimozide due to risk of drug interaction or QT c prolongation (4, 7.9 Thioridazine: PROZAC is contraindicated for u

22、se with thioridazine due to QT c interval prolongation or potential for elevated thioridazine plasma levels. 39、to placebo of suicidal thinking and behavior (suicidality in children, adolescents, and young adults in short-term studies of Major Depressive Disorder (MDD and other psychiatric disorders. 6 Anyone considering the use of PROZAC or any other antidepressant in a child, adolescent, or young adult must

40、 balance this 7 risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants 8 compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in 9 adults aged 65 and older. Depression and

41、 certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed 11 closely for clinical worsening, suicidality, or unusual changes in behavior. Families

42、 and caregivers should be advised of the 12 need for close observation and communication with the prescriber. PROZAC is approved for use in pediatric patients with 13 MDD and Obsessive Compulsive Disorder (OCD see Warnings and Precautions (5.1 and Use in Specific Populations (8.4. 14 When using PROZ

43、AC and olanzapine in combination, also refer to Boxed Warning section of the package insert for Symbyax. 16 1 INDICATIONS AND USAGE 17 1.1 Major Depressive Disorder 18 PROZAC is indicated for the acute and maintenance treatment of Major Depressive Disorder in adult patients and in 19 pediatric patie

44、nts aged 8 to 18 years see Clinical Studies (14.1. The usefulness of the drug in adult and pediatric patients receiving fluoxetine for extended periods, should periodically be re21 evaluated see Dosage and Administration (2.1. 22 1.2 Obsessive Compulsive Disorder23 PROZAC is indicated for the acute

45、and maintenance treatment of obsessions and compulsions in adult patients and in 24 pediatric patients aged 7 to 17 years with Obsessive Compulsive Disorder (OCD see Clinical Studies (14.2. The effectiveness of PROZAC in long-term use, i.e., for more than 13 weeks, has not been systematically evalua

46、ted in 26 placebo-controlled trials. Therefore, the physician who elects to use PROZAC for extended periods, should periodically re-evaluate 27 the long-term usefulness of the drug for the individual patient see Dosage and Administration (2.2.28 1.3 Bulimia Nervosa29 PROZAC is indicated for the acut

53、quate dose and duration in the current episode. 51 PROZAC monotherapy is not indicated for the treatment of treatment resistant depression. 52 2 DOSAGE AND ADMINISTRATION53 2.1 Major Depressive Disorder54Initial Treatment55 Adult In controlled trials used to support the efficacy of fluoxetine, patie

54、nts were administered morning doses ranging from56 20 to 80 mg/day. Studies comparing fluoxetine 20, 40, and 60 mg/day to placebo indicate that 20 mg/day is sufficient to obtain a57 satisfactory response in Major Depressive Disorder in most cases. Consequently, a dose of 20 mg/day, administered in t

55、he morning, is58 recommended as the initial dose.59 A dose increase may be considered after several weeks if insufficient clinical improvement is observed. Doses above60 20 mg/day may be administered on a once-a-day (morning or BID schedule (i.e., morning and noon and should not exceed a61 maximum d

56、ose of 80 mg/day.62 Pediatric (children and adolescents In the short-term (8 to 9 week controlled clinical trials of fluoxetine supporting its63 effectiveness in the treatment of Major Depressive Disorder, patients were administered fluoxetine doses of 10 to 20 mg/day64 see Clinical Studies (14.1. Treatment should be initiated with a dose of 10 or 20 mg/day. After 1 week at 10 mg/day, the dose65 should be increased to 20 mg/day.66 However, due to higher plasma levels in lower weight children, the starting and target dose in this group may be
