Everyyear,RolandBerger'sFutureofhealthstudyfocusesonatopicofparticularrelevanceforthehealthcaresector.Forthisyear'sstudy–thesixthinour
series–weconsulted100managersandboard-levelexecutivesin12countries,1combiningdetailedinterviewswithabroadersurveyofattitudesand
expectations.Ourfocuswasonatopicthathasdominatedthepublic
discourseacrosshealthcaresectorsinrecenttimes:artificialintelligence(AI),includinggenerativeartificialintelligence(GenAI).
AIisexpectedtobringarangeofbenefits:Somecommentatorsare
predictinggroundbreakingqualityimprovementsincare,whileothersforesee
unparalleledeconomicbenefits.AIislikelytoimpactbusinessmodelsand
processesacrossthehealthcaresector.Butwillthenewtechnologycause
arevolutioninhealthcareoragentlerprocessofevolution?Howareplayers
applyingthenewtechnologytodayacrosstheindustry–andhowdotheyseeitimpactingdifferenthealthcaresectorsandtypesofhealthcareorganizationsoverthenextfiveyears?
Inourinvestigationweconcludethat,overall,AIwillcausewhatcanbest
bedescribedastherapidevolutionofhealthcare.Thisfast-pacedprocessof
evolutionwillbringsubstantialchanges,butitwillallowenoughtimeforplayerstoformulatecarefulstrategicresponses.Someareasofhealthcarearealreadyseeingrapidtransformation:Inhospitals,forexample,52percentofrespondentsreportusingAIatleastpartiallyintheirstandardmedicaldiagnosticoperations,withanother32percentcurrentlypilotingsuchsolutions.However,otherareas
areprogressingmoreslowly:Inthefieldoftherapy,36percentofhospitalshavenotstartedusingAIatall.Thepatientjourneywillimprovethroughpersonalizedtreatmentratherthannewproducts.Joblandscapesareshifting,withtechnicalrolesfacingthemostdisruption.However,anumberoforganizationsarelaggingbehind,withonly29percentfeelingpreparedtoexploitAIforcompetitive
advantage.Tobridgethisgap,strategicpartnershipswithtechgiantsarefavoredby87percentofrespondentsoverin-housedevelopment.
Themessageforstakeholdersacrosstheindustryisclear:Embrace

fronts,fromashortageofskilledworkerstoincreasinglystrictregulationandever-expandingcomplexityinareassuchasinformationmanagement.Intensifyingcostpressure,partlytheresultofagingpopulationsinmanycountriesaroundtheworld,isforcingplayersinthehealthcareecosystemtoadaptandfindnewwaystoboosttheirproductivity.2Onesolutionthathasthepotentialtobeagame-changerliesinthewidespreadapplicationoftechnologybasedonartificialintelligence(AI).WithinthebroadercategoryofAI,generativeartificialintelligence(GenAI–seeDefinitions)standsoutasaparticularlypromisingsubfieldthatcanofferinnovativeapproachestotacklethesechallenges.
Manyhealthcareexecutivesarealreadyexploringthepossibilitiesofusingthesenewtechnologiesintheiroperations.4Accordingtoajointstatementbythreeglobal

AI(artificialintelligence)
Abranchofcomputersciencethataimstocreatemachinescapableofperformingtasksthatwouldnormallyrequirehumanintelligence,suchasvisualperception,speechrecognition,decision-makingandlanguagetranslation.
GenAI(generativeartificialintelligence)
Atypeofartificialintelligencethatcangeneratenewcontent(text,images,videos)andisassociatedwiththeriseoflargelanguagemodelsandchatbots,suchasChatGPT.3

leadersinhealthcarepublishedattheWorldEconomicForum,advancementsindataanalyticsandAIarehighlypromisingandAIwilllikelytransformhealthcaredelivery,drivegroundbreakinginnovationsinhealthresearchandimproveoverallhealthoutcomes.5ItisclearthatAIwillcausemajordisruptionintheindustry.Whatislessclear,however,iswhetheritwillcauseasuddenrevolutionormoreofasteadyevolution.
Ourdetailedresearchandconsultationswithindustryinsidersleadustobelievethat,overall,AIwillcausewhatcanbestbedescribedastherapidevolutionofhealthcare.AIisdevelopingbothdisruptively(arevolution)andincrementally(anevolution),dependingonthespecificareathatthenewtechnologyisappliedin.Already,itisevidentthatAIwillhaveamajorimpactonplayers'businessmodels,requiringanurgentresponse.YetmostorganizationsarestillapproachingAIfromtheperspectiveofhowtheycanuseittoimprovetheirownoperations,neglectingthebroaderperspectiveofhowitwilltransformtheecosystembeyondtheirownorganizationalboundariesinwaysthatwillultimatelybenefitpatients.
AIisrelevantforalltypesofplayersinthehealthcarevaluechain,frompharmaceuticalcompaniestoindividualhealthcareprofessionals.Weareseeingawiderangeofresponsesfromthoseplayers,fromhealthinsurancecompaniesthatalreadydigitalizedtheiranalysesandinternalprocessesfiveyearsback6tomedicaltechnology(medtech)playersthatarejuststartingtheirAIjourney.Thepast12to18monthshaveseenamajorsurgeinactivity,withnumerousprojectsandfeasibilitystudiesunderway,suchastheimplementationofAI-drivendiagnostictoolsinpartnershipwithlargetechprovidersandpersonalizedtreatmentplans.
Therecanbelittledoubtthatnowisthetimeforplayerstoact,notsimplyforthesakeofdoingsomethingbuttoensurethattheyreapthefullbenefitsoftheAItransformation.Forinstance,companiesusingAIforpredictiveanalyticsinpatientcarearealreadyseeing

improvedoutcomesandoperationalefficiencies.Onemightevenask:DocompaniesinthehealthcaresectorneedtoembraceAIinordertostaycompetitive?Theanswerisanemphaticyes:BusinessesthatfailtoadoptAIriskfallingbehindoninnovationandeffectiveness.
Inthis,oursixthFutureofhealthstudy,weagaintakeacross-sectoralapproach.WeexaminethepotentialofAIacrosstheentirehealthcareecosystem,offeringabroaderperspectivethanthattypicallyprovidedbystudies.InChapter2,weexaminehowAIisbeingappliedinthe
sectorandwhatpotentialsolutionsitoffers.InChapter3,wetakeacloserlookatitsimpactonthepatientjourneyanddifferentpartsofthehealthcarevaluechain.Next,weexamineitsimplicationsforhealthcareorganizations(Chapter4)anddeterminewhichscenariofortheadoptionofAIoverthenextfiveyearsismostlikely(Chapter5).Weconcludeourstudywithaseriesofrecommendationsforhowstakeholdersshouldpreparethemselvesforthetransformation–atransformationthatinmanyareasisalreadywellunderway.

neofthemainfactorsputtingpressureontoday's
healthcaresystemsistheincreasingprevalenceofobesity.Globallifeexpectancyincreasedbysixyearsoverthelasttwodecades,whilehealthylifeexpectancygrewbyjustfiveyears.7Thismeansthatpeopleonaverageareexperiencingmoreyearsofillhealththaninthepast.Anothercontributingfactoristheincreasingprevalenceofobesityworldwide.Preventivehealthcareisthereforebecomingmoreimportantthanever–andthisisanareawhereAIisapowerfultooltosystematicallyreduceratesofillness.Forexample,AIcanhelpidentifychronicconditionssuchascardiovasculardiseaseanddiabetes.A
Inparallelwiththegrowingdemandforhealthcareservices,thesectorisbesetbyalackofhumanandfinancialresources.Healthcareorganizationsarefacingaprojectedshortfalloftenmillionhealthworkers
worldwidebytheyear2030,mostlyinlowandlower-middleincomecountries.8Thisshortageofskilledworkersandhenceoverworkingofstaffcreatesanimbalancebetweensupplyanddemand.AImayholdthekeytomakingsystematicimprovementshere,bymakingroutinetasksredundant,butitsapplicationinthisareaisstillinitsinfancy.
Furthermore,patients'expectationsofhealth-careareincreasingastheybecomeaccustomedtoconvenienceinotherareasoflife.Yetpatientscontinuetofaceunequalaccesstoqualitycareandinconsistenthealthcareoutcomes.IntheUnitedStates,forexample,peoplelivinginruralareasdieatrates20percenthigherthanthoselivinginurbanareas,agapthathasbeenwideningoverthepasttwodecades.9Globally,thesedisparitiesareofteninfluencedbysocioeconomic

factors,discrimination,biasandunevendistributionofhealthcareresources.Differentsectorsalsofacespecificchallenges:Researchanddevelopment(R&D)facesrisingcosts,forexample,whilemedtechandpharmaceuticalcompaniessufferfromsupplychainvulnerability.Here,AIcouldcontributetobetterplanninganddistributionofresources.
Inconclusion,itisapparentthatthehealthcaresystemisoftenworkinginefficientlyandfailingtomeetpopulations'needsinanoptimalfashion.Thesituationisworseningovertime,sostandingstillactuallymeansgoingbackward.AIcouldbethesolution–butthequestionremains,how?
WHATCANAIDO?
Overthenextfivetosixyears,thehealthcaresystemisexpectedtoundergoamajortransformationthankstoadvancementsinAItechnologyandtheadoptionofAIsolutions.Already,inthepastthreeyears,investorshaveinjectedmorethanUSD30billionintohealthcareAIcompanies,makingthisoneofthemostheavilyfundedsectors.10Infact,theglobalhealthcareAImarketisexpectedtobeworthuptoUSD187.7billionby2030.11
AccordingtoanalyticsfirmResearchandMarkets,healthcarespendingonAI-relatedhardware(suchasspecializedprocessingchipsanddevicesthatusethem)andsoftwarefordiagnostics,imageanalysis,remotepatientmonitoringandthelikeamountedtoaroundUSD26billionin2024.ThisfigureisprojectedtorisetoanimpressiveUSD59billionby2028.12AnalystsatCBInsightsestimatethatoverhalfofthe1,500healthAIvendorscurrentlyoperatinghaveemergedoverjustthepastsevenyears.13
Largelanguagemodels(LLMs)andAIsystemsalreadyserveasthebackboneformanyhealthcare-specificapplications.Forexample,theyarewidelyusedformedicalresearch,chatbots,imagerecognition,administrativetasks,dataanalysis,diagnosticsand

treatmentplanning.Similarly,multimodalAIhealthcaremodelscanprocessandintegratedatafrommultiplesourcessuchastext,images,audio,videoandsensordata.Medicalimaginganalysis,forinstance,combinesdatafrommedicalimagingwiththepatient'shistory,geneticinformationandclinicalnotestogeneratemoreaccuratediagnosesandeffectivetreatmentdecisions.OneleadingEuropeanmedtechcompanyhastakenjustthisapproach,developingproprietaryAIsolutionsthatenhancethequalityofmedicalimageanalyses.Initiallybuiltontraditionalmachinelearningtechniques,thepioneeringcompany'ssolutionhasevolvedtoincorporatesophisticatedAI(andGenAI)algorithms,significantlyimprovingdiagnosticaccuracyandtreatmentefficacy.
ThehealthcareAIstartupmarketisdevelopingrapidly,withhundredsofstartupsemergingworldwide,asignificantportionofwhicharebasedintheUnitedStates.14EstablishedhealthcareenterpriseshavealsostartedincorporatingAIintotheiroperationsandarenowtryingtoscaleuptheseefforts.Forexample,modernprostheticscanuseAItoprovidemorenaturalmovementsandfunctions,learningandadaptingtotheuser'smusclemovementsandprovidingimprovedcontrolanddexterity. weseeanincreasingtendencyforlargehealthcareandpharmaceuticalcompaniestodeveloptheirownAItechnologies,ofteninclosecollaborationwithtechgiants.ExamplesincludethepartnershipbetweenAWSandalargeAmericanmultinationalpharmaceuticalcompany.GoogleCloud,ontheotherhand,hasestablishedseveralpartnershipswithsignificanthealthsystemsandhealthplansoverthelastfewyears,withtheaimofdevelopinggenerativeAIsolutionsandtoolstoreduceadministrativeburdens.Inbothcases,thecompanieshavetailoredthetechnologytotheirspecificusecaseandsocarvedoutacompetitiveadvantageforthemselves;indeed,theirimpactonefficiencyisevenmeasurableattasklevelnow.15Smallandmid-sizedfirms,ontheotherhand,aremorelikelytocollaboratewithspecializedAIprovidersorpurchaseAIsolutions,allowingthemtotapintoexternalexpertiseandtechnologywithoutmakingsignificantinternalinvestments.
ThehugecommitmentandlevelofinvestmentisevidencethatAIhasthepotentialtoaddressthesystemicchallengesfacingthehealthcaresystem,leadingtoaparadigmshiftinthesector.Therevolutionthatwilltakeplaceinsomeareaswillbehighlyvisibleforpatients,whilethemoregradualevolutionthatwilloccurinotherswilloftengounnoticed–especiallyifplayersdonotnecessarilywanttheircustomerstoknowthattheyareusingAI.B
However,thechangestohealthcaresystemscausedbyAIvarybyregionduetolocalregulations,dataprivacylawsandinfrastructure.Forinstance,theEuropeanUnion'sstrictGeneralDataProtectionRegulation(GDPR)impactshowhealthcareprovidershandleAI-relateddatainEurope,whileintheUnitedStatestheregulatorylandscapeisevolvingmorerapidly,enablingfasterimplementationofAIsolutions.16Patientpreferencesandbehavioralsodifferbyregion:Forexample,customersintheMiddleEastaregenerallymoreopentodigitalpatientsolutions.17

PotentialusecasesforAIarefoundacrosstheentirepatientjourneyandinallpartsofthehealthcarevaluechain.AIhasthepotentialtoaddressmanyofthemostpressingissuesinthecurrenthealthcaresystem.Forexample,itcanmitigatetheskillsshortagebyreplacingjobsandallowingemployeestoretrainforothertasks.Itcanreducethecostofhealthcarebydetectingillnessesearlyandpromptingproactiveintervention.Itcanimproveefficiencyinsimpleadministrationtasks,suchasclaimsmanagement,patientschedulingandbilling,freeingupstaffforothertasks.Anditisexpectedtounlocksignificantqualityimprovements–byenablingbetter,fasterdecisionsindiagnosesandtreatments,say.
ThankstoAI,patientjourneysareexpectedtobetransformed.Often,theywillbecomehybrid,combiningtraditionalin-personcarewithdigitalservicesenhancedbynewtechnology.AIenables24/7availabilityofhealthcareservices,regardlessofpatients'geographicallocation.Forexample,the"AIhospital"beingpilotedinChinaisavirtualhospitalruntoalargeextentbyAIphysiciansandnurses,showcasingthepotentialofAI-basedsolutionsforcontinuoushigh-qualitycare.18AI-generatedphysicianscouldpotentiallytreat10,000patientsinjustafewdays(excluding,ofcourse,moreadvanceddiagnosticcasesandcasesrequiringaphysicaltreatment),whereastheirhumanequivalentswouldneedatleasttwoyears.
AIalsoopensthedoortopersonalizedmedicinebyanalyzingpatient-specificdataandtailoringtreatments,improvingoutcomesandminimizingunnecessaryinterventions.Itsabilitytointegratedatafromvarioussources,suchaselectronichealthrecords,genomicsandwearables,meansthatitcanoffercross-functionalinsightstoimprovepatientcare.Notonlythat,AI-drivendecisionsupportsystemscanhelpclinicianschooseoptimaltherapiesandcreatecomprehensivecarepathways.Indeed,atamorebasiclevel,healthcarecanbecomepreventiveratherthanreactive.

TheboxAI–Sampleusecasesinhealthcarelistsawiderangeofpotentialusecasesforartificialintelligencerightacrossthehealthcareindustry–andtherearemanymore.However,thequestionremains:WhichareasofhealthcarewillbethemoststronglyaffectedbyAI?Inotherwords,wherewilltheuseofAIestablishitselfasthedefactostandard?
Expertsandhealthcareprofessionalsaredividedonthisquestion.Tofindouthowtheyfeelaboutthisandrelatedissues,weconsultedmorethan100board-levelexecutivesandalargenumberofhealthcareexpertsin12differentcountries:theUnitedStates,Germany,theUnitedKingdom,theUnitedArabEmirates,China,Norway,Japan,France,Sweden,Austria,SaudiArabiaand
Switzerland.Ourrespondentsrepresentedacross-sectionoftheindustry:31percentworkedforpharmaceuticalcompanies,24percentforhealthcareproviders,another24percentformedtechfirmsand20percentforhealthinsurancecompanies.Theorganizationsthattheyworkedforwereofarangeofdifferentsizes.Two-thirdsofrespondentsweremale,one-thirdfemale,andtheiraverageagewas44.
Inthefollowingtwochapterswepresentsomeofthemoststrikingresultsofthissurvey,focusingfirstontheimpactofAIonthepatientjourneyanddifferentpartsofthehealthcarevaluechain(Chapter3),andthenturningtotheimplicationsoftheAItransformationforhealthcareorganizations(Chapter4).

However,AIwillnotaffectallpartsofthehealthcareindustryequally.Forexample,pharmaceuticalcompanieswillexperiencetheAItransformationdifferentlyfromhealthinsurancefirms.Inthischapter,wesetouttofindoutwhichareasarealreadybenefitingfromAI-basedtechnology,andwhichareasarelikelytobenefitthemostgoingforward.Theseinsightscanhelpcompaniesfocusonthoseareaswherethemostvaluewillbeadded–andstartpreparingthemselvesnowfortheopportunitiesahead.

JOURNEYTHANCOMMONLYPERCEIVED
AlthoughAItechnologystillhassomethingofanoveltyvalueforthegeneralpublic,healthcareplayershaveinfactbeenusingitinpartsoftheiroperationsforsometimenow.Inthesurvey,allrespondentssaidthattheycurrentlyuseAI,with74percentsayingthattheydosofrequently.However,just15percentsaidthattheyhaveatleastpartiallyemployedAIsolutionsintheiroperationsacrosstheircompany.Todate,theimpactofAIhasbeenevolutionaryratherthanrevolutionaryinmanypartsofthepatientjourney,itsmaincontributionbeingtheoptimizationofprocessesandcosts.Thus,81percentofrespondentsbelievethatthebiggestbenefitofusingAIisthatitspeedsupprocesses,79percentthatitenhancesthequalityoftheworktheydoand77percentthatitreducescosts.

MostrespondentsbelievethatAIwillsignificantlyassistphysiciansindailytasks.Specifically,82percentbelievethatAIwillreducedecision-makingtime,whichisparticularlyvaluableinacutecaresettings.Another
