Abstract:Objective:ToevaluatetherightventricularfunctionoffetuseswithnuchalcordbyusingtherightventricularTeiindexandvenouscatheterizationindex.Methods:Inthisstudy,120fetuseswithnuchalcordwereselectedastheresearchobjects.TherightventricularTeiindexandvenouscatheterizationindexwereobtainedbyprenatalultrasoundexaminationandechocardiographyexamination,andtheircorrelationanalysisandPearsondialysisverificationwereperformed.Results:TheresultsshowedthattherightventricularTeiindexandthevenouscatheterizationindexwerepositivelycorrelatedinfetuseswithnuchalcord(r=0.72,P<0.001),andbothwerehighlycorrelatedwiththeriskofventriculardysfunction.Conclusion:TheapplicationoftherightventricularTeiindexcombinedwiththevenouscatheterizationindexcaneffectivelyevaluatetherightventricularfunctionoffetuseswithnuchalcord,improvetheaccuracyandreliabilityofriskprediction,andhelpfamiliesmakerelevantpreparationsandinterventions.

Oneofthesignificantrisksassociatedwithnuchalcordisfetalrightventricular(RV)dysfunction.FetalRVfunctioniscrucialinmaintainingadequatepulmonarycirculationandoxygenation.Therefore,anydysfunctioninRVcanleadtoseverecomplications,suchasfetalhypoxiaanddistress.Traditionally,RVfunctionisevaluatedusingvariousimagingtechniques,suchasultrasound,echocardiography,MRI,etc.However,thesetechniquesarenotalwaysaccurateandreliable.

RecentstudieshaveshownthatcombiningtheTeiindexandvenouscatheterizationindexcaneffectivelyevaluatefetalRVfunctionincasesofnuchalcord.TheTeiindexisaDoppler-derivedindexthatassessesthecombinedsystolicanddiastolicfunctionofthemyocardium.Thevenouscatheterizationindexisatechniquethatmeasuresthevenousbloodflowintheinferiorvenacava(IVC)andhepaticvein.Bycombiningthesetechniques,practitionerscanobtainacomprehensiveassessmentoffetalRVfunction.

ThecombinedTeiindexandvenouscatheterizationindextechniqueofferseveraladvantagesovertraditionalimagingtechniques.Firstly,itisnon-invasiveanddoesnotrequireanyspecialequipmentotherthanDopplerultrasound.Therefore,itislessexpensiveandmoreaccessibletoabroaderrangeofpatients.Secondly,itprovidesamoreaccurateandreliableassessmentofRVfunction.ThisisespeciallyimportantincasesofnuchalcordwheretheriskofRVdysfunctionishigh.Thirdly,itallowspractitionerstomakeriskpredictionsmoreaccurately,therebyimprovinghealthoutcomesforboththemotherandthebaby.

Inconclusion,thecombinedTeiindexandvenouscatheterizationindextechniqueisaneffectivewayofevaluatingfetalRVfunctionincasesofnuchalcord.Thistechniqueoffersseveraladvantagesovertraditionalimagingtechniquesandismoreaccurate,reliable,andcost-effective.Therefore,itisessentialforpractitionerstofamiliarizethemselveswiththistechniquetodetectanypotentialcomplicationsearlyandimprovehealthoutcomesforboththemotherandthebabyAdditionally,itisimportanttonotethatnuchalcordisnotalwaysacauseforconcernanddoesnotalwaysrequireintervention.Infact,upto37%offetuseshaveanuchalcordatsomepointduringgestation,andthemajorityofthesecasesresolveontheirownwithoutanycomplications.

However,incaseswherethenuchalcorddoesresultinfetaldistressorcompromisedRVfunction,timelyinterventionandmanagementcanmakeasignificantdifferenceinoutcomes.Thismayinvolveemergencycesareandeliveryorotherinterventionstorelievethecordandrestoreadequatebloodflowtothefetus.

Itisalsoimportantforhealthcarepractitionerstoeducateandcommunicatewithpregnantpatientsaboutthepotentialrisksandwarningsignsassociatedwithnuchalcord.Thiscanhelpempowerwomentoreportanyconcerningsymptomsorseekpromptmedicalattentionifnecessary.

Insummary,nuchalcordisarelativelycommonoccurrenceduringpregnancy,andinmostcases,itdoesnotposeasignificantrisktofetalhealth.However,incaseswhereitdoescausefetaldistressorcompromisedRVfunction,timelyandappropriatemanagementisessentialtoensurethebestpossibleoutcomesforbothmotherandbaby.ThecombinedTeiindexandvenouscatheterizationindextechniqueoffersa
