3、icable safety standards are complied with.2.0 scope2this procedure is directly applicable to all equipment (new, modified, altered, or rearranged) that may be used directly in the manufacture of hitachi gsp products.2.2 this procedure does not address equipment that is not hitachi gsp purchased and

4、owned.2.3 this procedure is not intended to be directly applicable to facilities equipment such as hvac, panel boards, load centers, air compressors, di water systems, process support equipment (like heat exchangers, hot water heaters, gas cabinets, clean air systems and other such support equipment

5、. however, facilities maintenance may use this procedure as part of their equipment installation program(s) as appropriate.2.4 equipment inspection and approval exemptions:2.4.1 all exempted equipment must:a. be approved by a recognized testing agency or built to an acceptable industry standard b. b

6、e battery operated or plug-in (either grounded using a three-wire attachment or double insulated with a conforming label).2.4.2 exempted equipment:a. plug-in tools which are normally serviced by complete replacement (e.g. lamps, projectors, desk calculators, individual recharging devices).b. tools,

7、equipment, and equipment accessories that do not present a significant safety, industrial hygiene, or environmental hazard and are not serviced or modified by hitachi gsp personnel (e.g. copier, microwave oven, solder iron, hand power tools, hand-held bar code scanners)-c. equipment specifically exe

8、mpted by ohs based on the suitability for use.d. relocated equipment which:a) is plug-in, andb) has an approved equipment approval tag, andc) does not have any hazard due to the relocation, andd) does not present a significant safety, industrial hygiene, or environmental hazard, ande) no modificatio

9、ns of any kind have been made to the equipment.3.0 definitions3conditional operational approval indicates the equipment and services (e.g.electrical, compressed air, exhaust, water) utilized by the equipment have been inspected for problems that would prevent safely energizing the equipment for debu

10、g work. once conditional operational approval has been granted, the equipment may be operated under engineering control, maintenance control, or by manufacturing under direct continuous supervision by engineering.3.2 operational approval indicates that the equipment meets the location health and saf

11、ety requirements. once operational approval has been granted, the equipment may be operated by the final user.3.3 equipment approval tag or sticker identifies the equipment and the responsible ohs reviewer. this tag or sticker is kept visible on the equipment. a red adhesive sticker is placed on the

12、 equipment approval tag when conditional operational approval is necessary.3.4 equipment engineering (or the individual seeking safety approval) is the party responsible for selecting/designing, purchasing/acquiring, and installing the equipment3.5 operating department manager manages the department

13、 whose employees will operate the equipment.3.6 hitachi gsp corporate standard nonproduct equipment design standard provides safety criteria for nonproduct equipment designed and/or built for internal use at hitachi gsp. the standard applied to the design of all tools, process and test equipmen匸 how

14、ever, the standard is not intended to apply to the design of commercial products or to facilities service and building fit-up equipment.4.0 responsibilities and authority4.1 equipment engineering4.1.1 ensure that the design and purchase of equipment meets applicable standards and list all applicable

15、 standards as part of the equipment specifications4.1.2 review the equipment proposal and design and note all deviations from the equipment specification. these deviations must be resolved with ohs prior to tool build.43 follow equipment inspection and approval procedure addressed in section 6.0.4.2

16、 procurement team4.2.1 assures that copies of applicable specifications accompany purchase order.4.2.2 requires vendors to conform to applicable specifications.4.3 operating department manager4.3.1 ensure a valid equipment approval tag or sticker is maintained and displayed on theequipment.43.2 ensu

17、re that equipment which does not have valid approval tag or sticker is not operated.4.3.3 ensure employees receive appropriate training for equipment operation and maintenance 4.3.4 correct the deficiencies and requesting safety review for equipment which has been determined to be unsafe.4.4 ohs tea

18、m4.4.1 schedule the requested review in a timely manner.4.4.2 inspect all new, relocated, (unless it meets an exemption listed in this procedure) or modified equipment for compliance with health and safety requirements.4.4.3 determine whether a more detailed hazard analysis is required for the equip

19、ment or associated process.5.0 reference documenthgsp ohs manualhitachi gsp corporate standard nonproduct equipment design standard60 procedure2.5 prior to requesting inspection, equipment engineering should ensure:6 1 the following items are in place (if required).-proper electrical connections, gr

20、ounding, fuse size, disconnects, interlocks and explosion proofing-adequate ventilation (all exhaust installations or modifications must follow the hitachi gsp industrial ventilation procedure)-adequate noise control, baffles and enclosures-required mechanical guarding, hose connection ratings, lagg

21、ing equipment to floor-adequate radiation shielding from x-rays, high intensity light sources, rf, and microwaves adequate pollution control and waste disposal equipment6.1.2 a copy of the operating procedures and lockout/tagout procedures are available for ohs.6j.3 equipment is ready for inspection

22、 (located in its intended location and all facilities utilities have been installed such as air, electrical power, gases, water etc.note: equipment engineering should notify ohs preferably 1 business day in advance of the needed inspection by giving the "equipment safety check sheet" (ohsr023) to ohs team.2.6 ohs conducts equipment inspection to hitachi gsp safety and health requirements. after inspection and review are complete, ohs will grant debug, operational, or conditional operati
