船/岸安全检查表SHIP/SHORESAFETYCHECKLIST船名Ship'sName:泊位Berth: 港口Port:抵港日期DateofArrival: 抵港时间TimeofArrival:填表说明/INSTRUCTIONSFORCOMPLETION:安全的作业需要考虑所有的相关要求，双方或单方应各自承担为遵守这些要求所需的责任和义务。无论哪一方如果未能准备好接受某一项要求，则应在“备注”栏里做好注释，并对作业是否能够进行给予相应的评估。当某一具体项被认为是不适用于船舶、码头或该作业时，应在“备注”栏里注明。虽然该船/岸检查表是基于货物作业制定的，船舶在码头进行洗舱作业时也建议采取同样的做法。在“代码”栏中出现的“A”、“P”和“R”的意思是：A（协商）—表示该条目应在检查表的“备注”栏里注明所达成的协议或议定的程序，或者以其它双方都认可的方式进行了沟通。P（许可）—对于标注“P”的条目是否定回答的情况下，未经主管机关的书面许可，不可进行作业。R（复查）—表示该条目在双方约定和声明陈述的时间间隔内，需要反复检查。联合声明只有当各方已经进行了检查并且接受相应的责任和义务后才可签署。Thesafetyofoperationsrequiresthatallrelevantstatementsareconsideredandtheassociatedresponsibilityandaccountabilityforcomplianceisaccepted,eitherjointlyorsingly.Whereeitherpartyisnotpreparedtoacceptanassignedaccountability,acommentmustbemadeinthe‘Remarks’columnanddueconsiderationshouldbegiventoassessingwhetheroperationscanproceed.Whereaparticularitemisconsideredtobenotapplicabletotheship,theterminalortotheplannedoperation,anotetothiseffectshouldbeenteredinthe‘Remarks’column.WhiletheShip/ShoreCheckListisbaseduponcargohandlingoperations,itisrecommendedthatthesamepracticeisadoptedwhenatankerpresentsitselfataberthfortankcleaning.Thepresenceoftheletters‘A’,‘P’or‘R’inthecolumnentitled‘Code’indicatesthefollowing:A(‘Agreement’).Thisindicatesthatthereferencedconsiderationshouldbeaddressedbyanagreementorprocedurethatshouldbeidentifiedinthe‘Remarks’columnoftheCheckList,orcommunicatedinsomeothermutuallyacceptableform.P(‘Permission’).Inthecaseofanegativeanswertothestatementscoded‘P’,nooperationsaretobeconductedwithoutthewrittenpermissionfromtheappropriateauthority.R(‘Re-check’).Thisindicatesitemstobere-checkedatappropriateintervals,asagreedbetweenbothpartiesandstatedinthedeclaration(Suggestthere-checkintervalsdonotexceed2hours).Thejointdeclarationshouldnotbesigneduntilallpartieshavecheckedandacceptedtheirassignedresponsibilitiesandaccountabilities.
