染预防与控制培训 掌握了重点部门医院感染防控的重点 会议期间还安排参观了湘雅医院的 ICU 胸外 血透室 内镜室 口腔门诊 病区的治疗及处置室等部门的实地查 看 学习了重点部门重点环节的建筑布局 消毒隔离措施 多重耐药菌的监测管理 层流手术室的管理 新生儿及 ICU 的医院感染管理等以及对重点部门的监测采样方法 口腔诊疗器械管理 以及今后工作中如何开展综合监测与 目标监测及手术部位感染等监测 2 在现有条件下 规范了供应室的器械清洗 消毒灭 菌流程 将各科使用的压脉带 湿化瓶等物品统一规范到 供应室处理 实行统一下收下发 改变了以往由科室自行 处理的流程 降低感染风险 3 规范医务人员手卫生 使职工在意识上树立了手卫 生 从无到有的意识观念 特别是手术室规范了洗手流程 改善了洗手的方法 从原来的肥皂洗手 改成抗菌洗手液 及感应式外科手消毒方法 为手术室制定了洗手的流程图 提高了洗手的正确率和合格率 每月进行手卫生抽查及监 测 4 为各科制定了各种消毒登记本 规范了各种消毒液 的配制及使用 规范了医疗废物分类收集及交接 制定统 一的登记本及医疗废物转移交接单 5 每季度 重点部门每月 对各科进行环境卫生学监 测 6 每月组织各科护士长召开院感反馈会议 对存在的问 题加以整改 计划每半年召开医院感染委员会议 今年 6 月 12 月 7 制定了年度每月培训计划 人 物 洁 污流线不得交叉 逆 行 新生儿科在房屋布局上 进入办公区应有进门的 缓冲间 医务人员进入办公区更衣 换鞋 外科洗手 设施 干手设施 新生儿入院通道和出院通道窗口 才能有效控制医院感染 2 层流手术室在搬迁启用时 要请疾控中心进行采样 监测合格 方可投入使用 特殊手术间眼科 手术台 工作面高度截面平均风速比其他手术室宜降低 1 3 一 级眼科专用手术室手术区每边不小于 1 2m 如果眼科 新生儿科等重点部门的医院感染进行控制 也 才能使医生合理应用抗生素 同时对抗生素的合理使 用及联合用药无法进行监测 并且也是创二甲复审必 备的条件 4 按照现行医院感染管理 建议在条件允许情况下 安装院感实时监测系统和供应室追溯系统 以便更好 地管理 5 在搬新区后 对重点部门如 ICU 手术室 新生儿 科的消毒等工作好与坏是预防控制医院感染的重要环 节 建议这些科室的保洁人员直接由科室监督管理 因为在这些科室如果保洁人员消毒隔离工作做不到位 最容易导致医院感染事件发生 如果仍然由保洁公司 吃大锅饭管理 是做不好医院感染防控这块工作的 这样存在的隐患大 不利于科室管理 6 层流净化设备的维护 应设专门维护管理人员定期 检查初 中 高级过滤器 新风过滤网清洗 1 次 月 粗效过滤器清洗 1 次 月 中效过滤器宜每周检查 3 个月更换一次等等的维护工作 实行感染 后勤 手 术室三位一体的管理 特别是暂存处必须远离 生活区 要做到五防 防鼠 防蟑螂 防蚊 防盗 防 渗漏 要具备冲洗消毒的条件 9 目前院感工作中的难点 检验科无法提供感染病例 病原学检查 无法对感染病例实施正确的治疗 临床医 生凭经验性用药 效果不理想 这也是临床医生在为病 人实施治疗中的困惑 请院领导在搬入新区后考虑这块 工作 通过学习 结合医院实际情况 我院院感工作几乎是从 零起 医务人员对院感意识观念淡薄
