typicallyreadmittedforsuchmedicalconditionsaspneumonia,heartfailureorbacterial

infections.Someofthesereadmissionsmayhavebeenunavoidableinanelderly,sick

population.Butmanycouldsurelyhavebeenpreventedthroughbetterplanningand

coordination.Themostdisturbingfindingwas(hathalfofthe

medical(nonsurgical)patien(sreadmittedwithin30dayshadnotseenaphysicianfor
