从LXRs-NF-κB通路探讨壮骨健膝方对KOA滑膜炎症影响的实验研究摘要:

目的:探讨壮骨健膝方对KOA滑膜炎症的影响及其机制。

方法:采用建立兔膝关节滑膜炎模型,将兔分为KOA组和壮骨健膝方组。利用ELISA法检测兔滑膜炎组织中IL-1β、TNF-α、IL-6等炎性细胞因子、LXRs、NF-κB及其下游基因表达水平。Westernblot法检测兔滑膜炎组织中IL-1β、TNF-α、IL-6、LXRs、NF-κB及其下游蛋白表达水平。

结果:壮骨健膝方组兔的滑膜炎症状有所缓解,IL-1β、TNF-α、IL-6等炎性细胞因子和NF-κB下游基因的mRNA和蛋白表达水平显著降低,而LXRs和NF-κB的蛋白表达水平显著升高。

结论:壮骨健膝方可以通过影响LXRs/NF-κB通路,调节炎性细胞因子的表达水平,从而减轻KOA滑膜炎症的程度。该方剂有望成为一种治疗KOA滑膜炎的新方法。

Abstract:

Objective:ToexploretheeffectsandmechanismsofZhuangguJianxiFangonsynovitisinkneeosteoarthritis(KOA)byaffectingtheLXRs/NF-κBpathway.

Methods:Arabbitkneesynovitismodelwasestablished,andtherabbitsweredividedintotheKOAgroupandtheZhuangguJianxiFanggroup.ELISAwasperformedtodetecttheexpressionlevelsofinflammatorycytokinessuchasIL-1β,TNF-α,IL-6,LXRs,NF-κB,anddownstreamgenesinsynovialtissuesfromrabbits.WesternblotwasusedtodetecttheexpressionlevelsofIL-1β,TNF-α,IL-6,LXRs,NF-κB,anddownstreamproteinsinsynovialtissuesfromrabbits.

Results:ThesymptomsofsynovitiswerealleviatedintheZhuangguJianxiFanggroup,andthemRNAandproteinexpressionlevelsofinflammatorycytokinesanddownstreamgenesofNF-κBweresignificantlydecreased,whiletheproteinexpressionlevelsofLXRsandNF-κBweresignificantlyincreased.

Conclusion:ZhuangguJianxiFangmayalleviatetheseverityofsynovitisinKOAbyaffectingtheLXRs/NF-κBpathwayandregulatingtheexpressionlevelsofinflammatorycytokines.Therefore,ZhuangguJianxiFangmaybeanewtreatmentforsynovitisinKOA.

OurresultsdemonstratedthatZhuangguJianxiFangsignificantlyreducedtheseverityofsynovitisinKOArats.Thiswasaccompaniedbyadecreaseintheexpressionlevelsofinflammatorycytokines,suchasinterleukin-1β(IL-1β)andtumornecrosisfactor-alpha(TNF-α),anddownstreamgenesofNF-κB,suchascyclooxygenase-2(COX-2)andmatrixmetalloproteinase-9(MMP-9).ThesefindingssuggestthatZhuangguJianxiFangmayexertanti-inflammatoryeffectsinKOAbyregulatingtheNF-κBpathway.

Furthermore,ourresultsrevealedthatZhuangguJianxiFangupregulatedtheexpressionlevelsofliverXreceptors(LXRs),nuclearreceptorsthatplayacriticalroleintheresolutionofinflammation.LXRsareknowntoinhibittheactivationofNF-κBandsuppresstheexpressionofinflammatorycytokines.Inthisstudy,increasedexpressionofLXRswasassociatedwithdecreasedexpressionofinflammatorycytokinesanddownregulationofNF-κBdownstreamgenes.TheseresultssuggestthatZhuangguJianxiFangmayalleviatesynovitisinKOAbyregulatingtheLXRs/NF-κBpathway.

Inconclusion,ourstudysuggeststhatZhuangguJianxiFangmaybeapromisingtreatmentforsynovitisinKOA.Theanti-inflammatoryeffectsofthistraditionalChinesemedicinemaybemediatedbytheregulationoftheLXRs/NF-κBpathwayandthedownregulationofinflammatorycytokines.FurtherstudiesareneededtoelucidatetheexactmechanismsbywhichZhuangguJianxiFangexertsitseffectsandtoevaluateitssafetyandefficacyinhumansubjectsFuturestudiescanalsoinvestigatethepotentialsynergisticeffectsofZhuangguJianxiFangwithothertraditionalChinesemedicinesorconventionalpharmaceuticalsintreatingKOA.Additionally,clinicaltrialsarenecessarytodeterminetheoptimaldosageandtreatmentdurationforZhuangguJianxiFang,aswellastoassessitslong-termsafetyandefficacy.

Furthermore,sinceKOAisamultifactorialdiseasethatinvolvesvariouscellularandmolecularpathways,itispossiblethatothersignalingpathwaysmayalsobeinvolvedintheanti-inflammatoryeffectsofZhuangguJianxiFang.Forexample,previousstudieshavesuggestedthattheMAPKandPI3K/AktpathwaysalsoplayaroleinthepathogenesisofKOAandmaybetargetedbytraditionalChinesemedicines(17,18).Therefore,itwouldbeinterestingtoinvestigatewhetherZhuangguJianxiFangaffectsthesepathwaysaswell.

Inconclusion,traditionalChinesemedicinehasbeenusedforcenturiestotreatamultitudeofailments,anditsanti-inflammatoryandanalgesiceffectshavebeenwell-documented.OurstudyprovidesevidencethatZhuangguJianxiFang,atraditionalChinesemedicineformula,maybeeffectiveintreatingsynovitisinKOAthroughitsregulationoftheLXRs/NF-κBpathwayandthedownregulationofinflammatorycytokines.Withfurtherresearch,thisformulamayprovetobeasafeandeffectivealternativetoconventionalpharmacologicaltherapiesforKOAInadditiontothepotentialuseoftraditionalChinesemedicineintreatingKOA,othernon-pharmacologicaltherapieshaveshownpromiseaswell.Exercise,forexample,hasbeenshowntobeeffectiveinreducingpainanddisabilityinpatientswithKOA.Specifically,low-impactexercisessuchaswalking,cycling,andwateraerobicscanhelptoimprovejointflexibilityandreduceinflammation.Physicaltherapy,includingmassageandjointmobilization,canalsobebeneficialinreducingpainandstiffness.

WeightmanagementisanotherkeyaspectofKOAmanagement.Maintainingahealthyweightcanreducethepressureplacedonthejoints,whichinturnreducespainandinflammation.Abalanceddiet,richinanti-inflammatorycomponentssuchasomega-3fattyacids,isalsoimportantinreducinginflammationandpromotingjointhealth.

Insomecases,surgerymaybenecessarytotreatadvancedKOA.Totaljointreplacement,inwhichthedamagedjointisreplacedwithanartificialjoint,isacommonprocedureforsevereKOA.However,aswithanysurgery,therearerisksandpotentialcomplications,anditshouldonlybeconsideredafterotherconservativetreatmentshavefailed.
