摘要:
目的:探究新诊断2型糖尿病(T2DM)患者神经电生理异常特点及相关因素。
方法:选取2018年至2020年在本院门诊就诊的50例新诊断T2DM患者作为研究对象,采用神经电生理学检查,比较其神经传导速度和电信号传导水平与正常组和糖尿病组的差异,并收集患者的临床资料,分析其神经电生理学检查结果与相关因素的关系。
结果:新诊断T2DM患者的神经传导速度、分泌电位幅值和潜伏期差异较大,与糖尿病组和正常组相比,差异有统计学意义。且神经电生理学检查结果与患者的年龄、病史、糖化血红蛋白水平等因素密切相关。
结论:新诊断T2DM患者神经电生理学检查异常存在特有的特点,其发生有多个相关因素的共同作用。

Abstract:
Objective:Toexplorethecharacteristicsofneuropathicelectrophysiologicalabnormalitiesandrelatedfactorsinnewlydiagnosedtype2diabetes(T2DM)patients.
Methods:FiftynewlydiagnosedT2DMpatientswhovisitedtheoutpatientclinicofourhospitalfrom2018to2020wereselectedasstudysubjects.Electrophysiologicalexaminationwasperformedtocomparethedifferencesinnerveconductionvelocityandelectricalsignaltransmissionlevelwiththenormalgroupandthediabetesgroup,andtocollecttheclinicaldataofpatients,analyzetherelationshipbetweentheelectrophysiologicalexaminationresultsandrelatedfactors.
Results:Thereweresignificantdifferencesinnerveconductionvelocity,secretedpotentialamplitude,andlatencybetweennewlydiagnosedT2DMpatients,thediabetesgroup,andthenormalgroup.Theresultsofelectrophysiologicalexaminationwerecloselyrelatedtotheage,history,glycosylatedhemoglobinlevelandotherfactorsofpatients.
Conclusions:NewlydiagnosedT2DMpatientshavespecificcharacteristicsofabnormalelectrophysiologicalexamination,anditsoccurrenceistheresultofthecombinedeffectofmultiplefactors.

Furthermore,theglycosylatedhemoglobinlevelwasfoundtobecloselyrelatedtonerveconductionvelocity,indicatingthateffectivecontrolofbloodglucoselevelsmaypreventordelaythedevelopmentofnervedamageinT2DMpatients.Theageanddiseasehistoryofpatientsshouldalsobetakenintoconsiderationwheninterpretingelectrophysiologicalexaminationresults.
Insummary,thisstudyprovidesimportantinsightsintotherelationshipbetweenelectrophysiologicalexaminationandT2DM.ThefindingssuggestthatelectrophysiologicalexaminationmaybeausefultoolinthediagnosisandtreatmentofT2DMpatients,andhighlighttheimportanceofearlydetectionandeffectivemanagementofbloodglucoselevelsinpreventingordelayingnervedamageinT2DMpatientsInadditiontothefindingsfromthisstudy,thereareseveralotherfactorsthatshouldbetakenintoconsiderationwheninterpretingelectrophysiologicalexaminationresultsinT2DMpatients.First,theseverityanddurationofhyperglycemiacanhaveasignificantimpactonnervefunctioninganddamage.Patientswhohavehadpoorlycontrolledbloodglucoselevelsforalongperiodoftimemayhavemoreseverenervedamagethanthosewhohavehadbetterglycemiccontrol.
Second,otherfactorssuchasage,gender,smokingstatus,andalcoholconsumptioncanalsoinfluencenervefunctioninganddamage.Forexample,olderpatientsandmalesmaybemoresusceptibletoneuropathy,whilesmokingandalcoholconsumptioncanworsennervedamage.
Third,certainmedicationsusedtotreatT2DM,suchasstatinsandmetformin,havebeenshowntohavepotentialneuroprotectiveeffects.ThesemedicationsmayslowtheprogressionofnervedamageandimprovenervefunctioninginT2DMpatients.
Finally,lifestylefactorssuchasexerciseanddietmayalsoplayaroleinnervefunctioninganddamage.RegularphysicalactivityandahealthydietmayhelptoimprovenervefunctioningandreducetheriskofdevelopingneuropathyinT2DMpatients.
Inconclusion,electrophysiologicalexaminationcanbeavaluabletoolinthediagnosisandtreatmentofT2DMpatients,providingimportantinformationaboutnervefunctioninganddamage.However,wheninterpretingtheseresults,itisimportanttoconsiderarangeoffactorssuchastheseverityanddurationofhyperglycemia,otherinfluencingfactors,andlifestylefactors.Bytakingacomprehensiveapproachtodiagnosis,treatment,andmanagementofT2DM,wecanworktowardspreventingordelayingnervedamageandimprovingtheoverallhealthandwellbeingofpatientsInadditiontomonitoringnervefunction,itisimportantforhealthcareproviderstoaddressotheraspectsofT2DMcare,suchasglucosecontrol,bloodpressuremanagement,andlipidmanagement.Thiscanbeachievedthroughacombinationofmedication,lifestylemodifications,andregularmonitoring.
MedicationisanimportantcomponentofT2DMmanagement,andavarietyofdrugsareavailabletohelpregulatebloodsugarlevels,bloodpressure,andbloodlipids.Thesemedicationscanincludeinsulin,oralhypoglycemicagents,beta-blockers,ACEinhibitors,andstatins.Healthcareproviderswillworkwithpatientstodeterminethebestcourseoftreatmentbasedontheirindividualneedsandhealthhistory.
LifestylemodificationsarealsocrucialformanagingT2DM.Thiscanincludedietarychanges,suchasreducingintakeofsugaryandprocessedfoodsandincreasingintakeoffruits,vegetables,andwholegrains.Regularexerciseisalsoimportantforimprovingbloodsugarcontrol,reducingbloodpressure,andpromotingoverallhealth.Healthcareprovidersshouldworkwithpatientstocreateapersonalizedplanforphysicalactivitythattakesintoaccounttheirindividualneedsandabilities.
RegularmonitoringisthefinalimportantcomponentofT2DMmanagement.Patientsshouldhaveregularcheck-upswiththeirhealthcareprovidertomonitorbloodglucoselevels,bloodpressure,andlipidlevels.Inaddition,patientsmayneedtoundergoteststomonitorkidneyfunction,eyehealth,andnervefunction.Bymonitoringtheseaspectsof
