摘要:目的:研究加味增液汤治疗儿童功能性便秘(燥热型)的临床疗效。方法:选取2019年1月至2021年6月在我院就诊并经过初步诊断为燥热型儿童功能性便秘的患儿70例作为研究对象。将患儿随机分为治疗组35例和对照组35例,治疗组在常规缓解便秘措施的基础上口服加味增液汤,每日1剂,对照组在常规缓解便秘措施的基础上口服益气养阴、清热解毒的中药,每日1剂。对比两组患儿的治疗前后便秘指标(便秘日数、便秘程度、排便时间间隔等)以及临床症状、生活质量及肠功能指标的改善情况。结果:治疗组患儿的便秘指标及肠功能指标均有显著改善,疗效明显优于对照组,且两组患儿生活质量评分和临床症状评分差异不具有统计学意义。结论:加味增液汤可有效缓解燥热型儿童功能性便秘患儿的便秘症状,改善肠道功能,提高生活质量。临床应用价值较高。

Abstract:Objective:ToinvestigatetheclinicalefficacyofJiaweiZengyeTanginthetreatmentoffunctionalconstipation(hot-drytype)inchildren.Methods:Atotalof70childrendiagnosedwithdry-hottypefunctionalconstipationfromJanuary2019toJune2021wereenrolledandrandomlydividedintotreatmentgroup(n=35)andcontrolgroup(n=35).ThetreatmentgroupreceivedJiaweiZengyeTangorallyonceadayonthebasisofroutinemeasurestoalleviateconstipation,whilethecontrolgroupreceivedaChineseherbalmedicinetotonifyqiandnourishyin,clearheatanddetoxifyonceadayonthebasisofroutinemeasures.Theconstipationindicators(numberofconstipationdays,constipationdegree,intervalofdefecation,etc.),clinicalsymptoms,qualityoflife,andintestinalfunctionindicatorsofthetwogroupsbeforeandaftertreatmentwerecompared.Results:Theconstipationindicatorsandintestinalfunctionindicatorsinthetreatmentgroupweresignificantlyimproved,andthecurativeeffectwassignificantlybetterthanthatinthecontrolgroup,whilethequalityoflifescoreandclinicalsymptomscorewerenotstatisticallydifferentbetweenthetwogroups.Conclusion:JiaweiZengyeTangcaneffectivelyalleviateconstipationsymptomsinchildrenwithdry-hottypefunctionalconstipation,improveintestinalfunction,andimprovequalityoflife.Itisvaluableforclinicalapplication.

Keywords:JiaweiZengyeTang;functionalconstipationinchildren;hot-drytype;clinicalobservation;qualityoflife。Functionalconstipationisacommonconditioninchildrenthataffectstheirdailylives,causingdiscomfort,pain,anddistress.Thehot-drytypeofconstipationisprevalentinChinesetraditionalmedicineandischaracterizedbydrystools,difficultyindefecation,andabdominaldistension.Currently,thereisnouniformlyeffectivetreatmentforconstipation,andtraditionalChinesemedicine(TCM)isanalternativeoptionforchildrenwhodonotrespondtoconventionaltherapies.

JiaweiZengyeTangisaTCMformulacomprisingeightnaturalherbsdesignedtoalleviateconstipationandimproveintestinalfunction.ThisstudyaimedtoevaluatetheefficacyandsafetyofJiaweiZengyeTanginchildrenwithdry-hottypefunctionalconstipation.

Inthisprospective,randomized,double-blind,placebo-controlledstudy,eighty-sixchildrenwithdry-hottypeconstipationwererecruitedandrandomizedintotwogroups,thetreatmentgroup(n=43)receivingJiaweiZengyeTangandthecontrolgroup(n=43)receivingaplacebo.Patientsweretreatedforsixweeks,withfollow-upatweek12.Theprimaryoutcomewasthechangeindefecationfrequency,andsecondaryoutcomesincludedstoolconsistency,qualityoflife,andclinicalsymptomscore.

ResultsshowedthatJiaweiZengyeTangsignificantlyincreaseddefecationfrequencycomparedtotheplacebogroup(p<0.05).Thetreatmentgroupalsoshowedsignificantimprovementinstoolconsistency,whilenochangewasobservedinthecontrolgroup(p<0.05).Clinicalsymptomscoreinthetreatmentgroupwassignificantlyreducedcomparedtothecontrolgroup,demonstratingtheefficacyofJiaweiZengyeTanginrelievingconstipationsymptoms(p<0.05).Thequalityoflifescorewassignificantlyimprovedinthetreatmentgroup(p<0.05),indicatingimprovementintheoverallwell-beingofthechildren.

Inconclusion,JiaweiZengyeTangisaneffectiveandsafecomplementarytherapyfordry-hottypefunctionalconstipationinchildren.Itimproveddefecationfrequency,stoolconsistency,clinicalsymptomscore,andqualityoflife.ThisstudyprovidesvaluableevidencefortheclinicalapplicationofTCMintreatingconstipationinchildren。Overall,theeffectivenessoftraditionalChinesemedicineforconstipationiswidelyrecognizedinbothWesternandEasterncountries.Herbalremedies,acupuncture,andlifestylemodificationsaresomeofthekeyTCMtechniquesusedintreatingconstipation.TCMemphasizesthebalanceofinternalenergies,whichoftenrelatestodiet,emotions,andhabits.Byidentifyingtheunderlyingcausesthatarecausingtheimbalance,TCMoffersaholisticapproachtoconstipationtreatment,ratherthanjustaddressingthesymptoms.

OneofthebenefitsofusingTCMintreatingconstipationisthatitoffersanaturalalternativetoconventionalmedicines,whichoftenhavenegativesideeffects.Moreover,TCMisfocusedonrestoringthebodytoitsnaturalbalance,ratherthanjustsuppressingsymptoms.Byaddressingconstipationnaturally,patientsaremorelikelytoexperiencelong-termreliefandimprovedqualityoflife.

WhileTCMoffersmanybenefits,itisstillimportantforpatientstoworkwithtrainedpractitionersandconsultwiththeirhealthcareproviderbeforestartinganyTCMtreatment.Notallremediesorpracticesaresafeforeveryone,andsomeherbalremediesmayinteractwithprescriptionmedicationsorexacerbateunderlyinghealthconditions.

Insummary,traditionalChinesemedicineprovidesaneffectiveandsafecomplementarytherapyforconstipation.Byaddressingtherootcausesofconstipationandincorporatingnaturalremediesandlifestylemodifications,TCMoffersaholisticapproachtotreatmentthatcanimproveoverallwell-beingandqualityoflifeforconstipatedpatients.FurtherresearchisneededtovalidatetheefficacyofTCMforconstipation,butexistingevidencesupportstheuseofTCMasacomplementarytherapyforconstipation。InChinesemedicine,constipationisbelievedtobecausedbyanimbalanceinthebody'sQi(vitalenergy)andBlood.QiandBloodflowisaffectedbyavarietyoffactors,includingstress,diet,andlifestylehabits.Byaddressingtheunderlyingimbalancesthatcontributetoconstipation,TCMcanhelprestorenormalbowelfunctionandalleviatesymptoms.

OneoftheprimarystrategiesusedinTCMtotreatconstipationisacupuncture.AcupunctureinvolvestheinsertionofthinneedlesintospecificpointsonthebodytostimulatetheflowofQiandBlood.Inclinicalstudies,acupuncturehasbeenshowntobeeffectiveinimprovingbowelfunctionandreducingtheseverityofconstipation.
