12、范，为加强工程爆破设计与施工组织的科学性、严密性，规范爆破安全评估管理行为,确保爆破作业的安全与质量，特结合我省行业实际制定爆破安全评估管理试行办法（简称试行办法）。第二条  爆破安全评估应由具备相应资质等级和作业范围的爆破作业单位承担。承担安全评估的单位不得在本项目中同时承接爆破设计施工、实施安全监理，不得委托有利害关系的爆破作业单位进行安全评估。第三条  爆破安全评估人员应持有爆破工程技术人员安全作业证，且作业级别和作业范围符合评估项目要求。第四条  爆破安全评估单位和人员，必须以国家法律法规、行业标准和规范为依据，坚持“理论科学

13、性、实践指导性、技术安全性、措施可靠性、评估严肃性、专家权威性”原则，认真贯彻“严格审慎、程序规范、集体评议、科学决策”的工作方针，实事求是，公平公正地进行安全评估。第五条  爆破安全评估工作应接受公安机关和行业协会监督。对违规评估的单位和人员，爆破协会及时给予通报批评，报请市级公安机关暂停爆破作业项目审批，情节严重的责令停业整顿，并作不良记录备案；违规评估引发重大安全责任事故的，由公安机关依法追究评估单位法律责任。 第二章  爆破安全评估的范围和内容 第六条  需要进行安全评估的爆破作业项目：1、在城市、风景名胜

14、区和重要工程设施附近实施爆破作业的；2、除上述范围内的爆破作业项目，业主提出需要进行安全评估的。第七条  爆破安全评估单位根据爆破安全规程（GB6722），对爆破作业项目的等级及安全性进行评估，并形成书面报告。若评估单位认为评估项目等级不合适或爆破设计施工方案不可靠，应当发回重新设计。第八条  爆破安全评估应包括下列主要内容：1、爆破作业单位的资质是否符合规定；2、爆破作业项目的等级是否符合规定；3、设计所依据的资料是否完整；4、设计选择方案是否可行；5、设计方法、设计参数是否合理；6、起爆网路是否可靠；7、存在的有害效应及可能影响的范围是否全面；8、保

15、证工程环境安全的措施是否可行；9、制定的应急预案是否适当。第三章  爆破安全评估基本步骤 第九条  按市场化原则，实施爆破作业单位自主选择符合相应爆破资质等级和作业范围的评估单位。第十条  A、B级爆破工程的安全评估应至少有3名持证的爆破工程技术人员参加。其中，A级爆破作业项目的第一评估人必须持有相应高级作业证和作业范围的人员担任；B级（含）以下爆破作业项目的第一评估人必须持有相应中级（含）以下作业证和作业范围的人员担任。环境十分复杂的重大爆破工程应邀请专家组咨询，并在专家组咨询意见的基础上，编写爆破安全评估报告。爆破安全评

16、估报告内容应当翔实，结论应当明确。第十一条  爆破作业单位应向评估单位提交申请爆破作业项目安全评估委托书（样表一），附工商营业执照、爆破作业单位许可证、爆破工程技术人员安全作业证、爆破作业人员许可证、爆破技术设计方案与爆破施工组织方案和本项目批文及合同等有关资料复印件。第十二条  爆破安全评估的基本步骤：（一）初审资料。评估单位与委托单位签订安全评估合同后7日内，完成对资料的合法性、有效性和完整性的审核，对评估资料不全的应通知委托单位予以补充。（二）踏勘现场。评估单位组成评估组，并确定至少1名项目评估技术负责人到爆破现场实施勘察，重点了解爆破物特征以及爆

17、区周边的环境、设施、布局（包括地面、地下、空中）、距离等安全状况。（三）召开评估会。经评估单位和委托单位协商，确定评估时间、地点，届时邀请项目建设单位（业主）参与，必要时可邀请作业项目所在地公安机关以及各级爆破协会派员出席。评估意见应形成书面材料，由评估人员逐一签名，加盖评估单位公章，并制作爆破作业项目安全评估报告书（样表二）交委托单位。第四章  爆破安全评估费用 第十三条  爆破安全评估取费原则，应以爆破工程消耗量定额和爆破工程综合单价为依据，结合爆破市场行情，以能支持进行评估所发生的费用为基础，加上人工、材料、设备、勘测、管理等各项开支消耗

18、及技术含量、评估风险等因素合理确定取费标准，评估费用最高不宜超过爆破作业项目总造价的5%。第十四条  爆破安全评估费应纳入项目建设单位（业主）工程费用。 第五章  附 则 第十五条  在爆破安全评估活动中，委托单位应为评估单位提供必要的工作方便与配合，其他被邀请的各方代表也应协同做好评估工作。第十六条  本试行办法经湖北省工程爆破协会第一届六次会长办公会审议，第一届理事会2014年年会（即第五次全体会议）通过。第十七条  本试行办法已报湖北省公安厅治安总队审核、批复
